Please note that reception staff are not able to answer questions by phone about new client/patient applications.

 

If you have previously filled out the Google Form, you will receive a response in the order in which we received your information. If you have filled out the form more than once, incompletely answered the questions, or called to check on the status of your request, there will be additional delays.
